Case Management is a collaborative process which assesses, plans, implements, co-ordinates, monitors and evaluates the options and services required to meet an individual’s health, care, educational and employment needs using communication and available resources to promote quality cost effective outcomes”  (Case Management Society UK Standards of Practice, 2005).

Goal Manager® enables case managers to fulfil these key responsibilities more efficiently by automating several essential processes, and facilitating collaboration between the patient and the rehabilitation providers.

  • Goal Manager® uses the ‘International Classification of Functioning, Disability and Health’ (ICF) – an internationally recognised classification system for patient functioning. By co-ordinating the data provided from the entire rehabilitation team in to one core assessment, Goal Manager® is able to identify and prioritise areas of most need.

  • Goal Manager® automates best-practice processes in goal setting including Goal Attainment Scaling (GAS) and the use of SMART objectives. Goals are stored and organised where they can be easily accessed, tracked and monitored by any member of the team to provide feedback on progress over time.

  • Goal Manager® facilitates comprehensive and up-to-date communication within the team by providing space to record progress on goals. Remote cloud-based access allows team members to view goal progress in real time, wherever they are in the world.

  • Goal Manager® informs discharge planning by coordinating detailed goal data across physical, psychological, social, emotional and vocational domains, identifying the clinicians that need to be involved in easy-to-access summary diagrams. The use of a functional assessment and tracking of goal attainment provides clear rationale for rehabilitation actions.

  • Goal Manager® contributes data for other case manager documentation, such as evidence towards educational and social care assessments (EHCP, PIP applications etc.), as well as providing quick and readily available feedback to commissioning bodies on client progress to ensure ongoing funding of services.
